Dear Heritage 5th Grade Families:

Thank you for taking the time to fill out these forms as they will help inform our classroom placements for the 2021-22 school year.

Many factors are taken into account when creating classroom assignments.  Our primary concern is creating well-balanced classrooms that create optimal learning opportunities for all children.

The Learning Management Form is required for all families to fill out as it will help us determine Summit and Google Classroom sections.  Please fill this form out by Sunday, April 25th at 11:59 pm.  If you do not fill this out, we will assume that there is no preference for the LMS.

The parent/guardian input form gives us more information about your child.  This form is optional. Here is the link to the parent/guardian input form and will close on May 28, 2021.
